Tough to pick an example because they are all so bad. If it helps, I used this format:

I'm sorry for {specific action} that hurt you and I've taken these {specific actions} to ensure that it won't happen again and I'd like to do this {specific action} to see if I can repair the harm and if I can't, please forgive me.

There's nearly a thousand and I've put them in chronological order starting with when you were four. (I pray to God you can't remember anything before that.)
